Papa’s Social Drivers of Health (SDoH) Navigation program leverages its national network of in-person Papa Pals, remote team of social care navigators, and best-in-class technology platform to identify and resolve members’ unmet social needs. With this program, health plans can expect to gain a better understanding of the root cause of their members’ needs and see more successful connections to community resources, as well as higher resolution of health-related social needs. The result? Greater health equity and lower costs.

Papa’s SDoH Navigation program is part of a new set of offerings, built around configurable, well-defined objectives that Papa has proven it can impact and directly attribute to its program.
